Detaining and separating children is traumatizing and unnecessary. Children and families do not belong in detention centers. The American Academy of Pediatrics has warned that even a few days in detention has long-lasting impacts on childrens mental and physical health.

As a medical doctor and board-certified pediatrician, I am writing to express my concern about the irreparable harm that would be caused to children by the proposed changes regarding the Flores Settlement Agreement. I am a graduate of Michigan State University College of Human Medicine, completed pediatric residency training at the Northwestern University Feinberg School of Medicine, and am a member of the American Academy of Pediatrics as well as faculty at both Northwestern and the University of Chicago Pritzker School of Medicine. I care for hospitalized infants, children, and adolescents regardless of their background, nationality, documentation status, insurance status, or ability to pay for services. I stand with over 60,000 pediatricians and countless other child health providers nationwide in opposing this proposed rule change. The proposed changes are in conflict with the Flores Settlement Agreement, current medical knowledge and research on child development, and the American Academy of Pediatrics policy statement on Detention of Immigrant Children. The protections for immigrant children under the Flores Settlement Agreement are vital to ensure that this vulnerable group is protected from further trauma. Many children arriving to our border are fleeing violence and unrest. The current standard for children to be held in the least restrictive environment supports age-appropriate development and fosters healthy attachment and resilience in the face of adversity (Ross A. Thompson, Early Attachment and Later Development, Handbook of Attachment: Theory, Research and Clinical Applications, ed. Jude Cassidy and Phillip R. Shaver, 1999, 265-286). Very young children are especially impacted by the presence of secure, reliable caregivers and environments - an impossibility in either separate child detention or family detention. Robust literature over the last several decades has established that traumatic experiences in early life - also known as Adverse Childhood Experiences, or ACEs - are highly correlated with later poor health outcomes including substance abuse, depression, cancer, stroke, diabetes, and early death (Initial landmark study: Felitti VJ et al. Relationship of childhood abuse and household dysfunction to many of the leading causes of death in adults. The Adverse Childhood experiences (ACE) study. The studied ACEs include emotional neglect and parental separation, inherently part of child detention, as well as physical abuse, sexual abuse, and physical neglect, all of which have been reported in United States detention centers. Notably, parental incarceration is included - incarceration or detention of a child is certainly just as harmful. Many immigrant children will remain in our country, and in addition to being morally objectionable, worsening their later health outcomes is economically short-sighted. Many of the long-term health consequences of the childhood trauma caused by this policy (stroke, diabetes, cancer) are the sources of our countrys largest medical expenditures. Wherever immigrant children eventually make their homes, they deserve protection and safety while in the care of the United States government. I have cared for medically complex children undergoing the asylum application process with their mothers at bedside, having travelled thousands of miles facing countless challenges to flee violence and obtain a better life for themselves and their families. It is challenging to provide appropriate care to developmentally impaired or medically complex immigrant children even in a hospital-based setting, due to language barriers, limited resources, and lack of information about previous care.
